Please read through the statements below and indicate your acknowledgement and agreement by checking each statement.

I acknowledge that the submission of this form indicates interest only and that I/we are not confirmed GO Team members until notified by Calvary Church

2. *

If accepted, once notified, I will submit a full GO Team Member application for each team member on or before Nov 30, 2025. Your email notification will contain the application link.

3. *

If accepted, I agree to submit a $100, non-refundable, deposit per person with the submission of each GO Team Member application.

4. *

I acknowledge that any remainder toward the total amount of $600 per person is due on or before Dec 31, 2025. No exceptions. Failure to submit full payment results in trip forfeiture. Deposits will not be refunded.

I acknowledge that each family member must have their own passport secured by Dec 31, 2025. (NOTE: the cost of the passport is not included in the trip cost.)
